Our story
A Brookfield practice, passed from doctor to doctor
For decades, Maple Medical Center has been the doctor down the street for Brookfield families. The building and the names have changed. The idea hasn’t: personal care, one patient at a time.
- The beginning
Dr. Arthur Appleyard Sr. opens the practice
Maple Medical Center began as the practice of Dr. Arthur Appleyard Sr. Later, Dr. Arthur Appleyard Jr. carried the practice forward for decades. The sign outside still reads “Since 1966.”
- 1997
Dr. Ghani joins
Dr. Ehtesham Ghani, an internist trained at the University of Illinois, joins the practice.
- 2003
A new home at 9223 Ogden
The practice moves into the red-brick building at 9223 Ogden Avenue, where it is today.

Dr. Ghani and Dr. Simrun Gill care for adults and older adults. Most new patients still come to us because a neighbor, friend or family member sent them.

If you ever need the hospital
Dr. Ghani is affiliated with UChicago Medicine AdventHealth La Grange, and both of our doctors are part of Loyola Physician Partners, the physician network of Loyola Medicine, which includes MacNeal Hospital in Berwyn.
If you are admitted, a hospital doctor (a hospitalist) may also see you. Our doctor and the hospital team stay in touch about your stay, and after you go home you come back to see your Maple doctor.

Do you accept Medicare?
Yes. Maple Medical Center accepts Medicare, and many Medicare Advantage plans. We are not in network with every plan, so call (708) 485-4050 with your plan name and we will check it before your visit. See Insurance and Medicare.

Are you accepting new patients?
Yes. Call our office to set up your first visit. We will check your insurance and tell you what to bring.
Who will I see at my visit?
You will see one of our internal-medicine doctors, Dr. Ehtesham Ghani or Dr. Simrun Gill. Both care for adults and older adults. Our nurse and medical assistants help with vital signs, tests and vaccines.
How often should an older adult see the doctor?
Most older adults should see their primary care doctor at least once a year, and more often if they have conditions like diabetes, high blood pressure or heart disease. Medicare covers a yearly wellness visit to plan your preventive care. Your doctor will suggest a schedule that fits your health.
Can I walk in?
Our building sign says walk-ins are welcome, but doctors’ schedules change through the week. Please call (708) 485-4050 first so we can make sure a doctor is available when you arrive.
