Insurance and Medicare
We accept Medicare and many insurance plans, but not every plan. The quickest way to know if we take yours is to call us before your visit. It only takes a few minutes, and it saves surprises.
- Original Medicare accepted
- Many Medicare Advantage plans
- Self-pay welcome

Does Maple Medical Center take my insurance? We accept Original Medicare and many commercial, HMO, PPO and Medicare Advantage plans, but we are not in network with every plan. Networks change every year, so please call (708) 485-4050 with your plan name and member ID before your visit, and we’ll check it for you.
The short version
Yes, we accept
- Original Medicare (Part B)
- Many Medicare Advantage plans
- Many commercial, HMO and PPO plans
- Self-pay patients
Please call first if you have
- A Medicare Advantage HMO
- A Medicaid or HealthChoice Illinois plan
- A Marketplace (Get Covered Illinois) plan
- A new plan for 2027, or a plan you just switched to
We’d rather tell you on the phone than at the front desk. That’s why we ask everyone to call first.
How to check your insurance in three steps
Find your card
Your insurance card or red-white-and-blue Medicare card. If you have a Medicare Advantage or supplement plan, have that card too.
Call (708) 485-4050
Tell us your plan name, member ID and group number. If it’s an HMO, tell us that too.
We confirm before your visit
We check whether we’re in network and let you know. If we’re not, we’ll explain your options.
Plans that list our doctors
We checked where Dr. Ghani and Dr. Gill are listed as in-network today: the Loyola Medicine and UChicago Medicine AdventHealth physician directories, and 2025–2026 directories from Molina, Cigna HealthSpring and Blue Cross. Directories can be out of date, and a listing is not a guarantee, so please call us to confirm your exact plan.

If you have Medicare
Original Medicare. We accept Original Medicare. Medicare Part B helps pay for doctor visits, many lab tests and preventive care such as your yearly wellness visit and many vaccines. Some Part B services have a deductible or coinsurance.
Medicare Advantage. These plans (also called Part C) are run by private insurers and have their own networks. We accept many of them, but not all. Call us with your plan name.
Supplement (Medigap) plans work alongside Original Medicare and can help with costs Medicare doesn’t pay. Bring that card too.
Changing plans? Medicare Open Enrollment runs October 15 to December 7. If keeping your Maple doctor matters to you, call us before you choose a new plan.

Copays, deductibles and self-pay
Insurance is designed to cover most costs. If your plan has a copay or deductible, please be ready to pay it at the time of service.
No insurance, or a plan we don’t take? We also see self-pay patients. Ask our office about costs when you call, before your visit.
Insurance questions
Do you accept Medicare?
Yes, we accept Original Medicare. We also accept many Medicare Advantage plans, but not all of them. Call us with your plan name before your visit.
My insurance company says you're in network, but is that enough?
It’s a good start. Plan directories are not always up to date, and some plans have several networks with similar names. A short call to our office at (708) 485-4050 confirms it from our side too.
What if you don't accept my plan?
We will tell you before your visit. You can choose to be seen as a self-pay patient, or look for an in-network doctor through your plan. We’d rather you know up front.
I have an HMO. Do I need to do anything first?
Usually, yes. Most HMO plans ask you to choose a primary care doctor before your first visit. Call your plan to name your Maple Medical Center doctor, and call us so we can check that we’re listed.
Do I pay anything at the visit?
If your plan has a copay or deductible, please be ready to pay it at the time of your visit. If you have questions about what you owe, call our office before you come.
I'm changing Medicare plans this year. When should I call you?
Before you choose. Medicare Open Enrollment runs October 15 to December 7 each year. If staying with your Maple doctor matters to you, ask us which plans we accept before you switch.
Do you see patients without insurance?
Yes, we see self-pay patients. Ask our office about the cost of your visit when you call.
