Getting started

  1. Call our office

Call (708) 485-4050, or send a request online and we’ll call you back.

  2. We check your insurance

We accept Medicare and many plans, but not every plan. We’ll check yours first.

  3. Send your records

Ask your previous doctor to fax your records to (708) 485-9216.

  4. Come to your first visit

Bring the items on the list below. Plan to talk about your health history, your medicines and your goals.

What to bring

  • Your insurance card, and your Medicare card if you have one
  • A photo ID
  • All your medicines in a bag, including vitamins, supplements and eye drops, or a complete list with doses
  • Names and phone numbers of your other doctors
  • Recent test results or hospital papers, if you have them
  • Your reading glasses and hearing aids
  • A list of questions you want to ask

Your medicines: the most useful thing you can bring

Put every medicine you take in a bag and bring it, including vitamins, supplements, inhalers, creams and eye drops. Older adults often take several medicines from several doctors, and a full review can catch doubled doses or medicines that don’t mix well.

New patient questions

How do I become a patient?

Call (708) 485-4050. We’ll check your insurance, find a time for your first visit and tell you what to bring. You can also send us a request online and we’ll call you back.

How do I send my old medical records?

Ask your previous doctor’s office to fax your records to us at (708) 485-9216. They will usually ask you to sign a release form first.

Can a family member come with me?

Yes. Many of our patients bring a spouse, son, daughter or caregiver. It helps to have a second set of ears.

Do you see children?

We are an internal-medicine practice and care for adults and older adults. Please call to ask about younger patients.
