If you’re admitted
Tell the hospital who your doctor is
Give them our name and number: Maple Medical Center, (708) 485-4050. It helps the hospital team reach us.
A hospitalist may care for you
Hospitalists work only in the hospital, so they can often see you more than once a day if needed.
We stay in touch
Our doctor and the hospital doctor keep each other informed about your stay.
You come back to us
After you’re discharged, you return to see your Maple doctor for follow-up.
After you go home
The first weeks after a hospital stay matter. Medicines often change, and new symptoms can show up. A follow-up visit with your own doctor helps catch problems early.
- Call us to book a follow-up visit soon after discharge
- Bring your discharge papers and every medicine you came home with
- Bring your old medicines too, so we can sort out what to keep and what to stop
- Bring a family member or caregiver if that helps you
- Write down questions as they come up at home
